HMC offers integrated care services for elderly patients

Hamad Medical Corporation (HMC) offers a wide range of geriatric healthcare services to promote the health and well-being of Qatar’s elderly population.

Dr.  Amir Ibrahim Abdalla (pictured), Consultant Geriatrician and Emergency Geriatrics Clinical Lead at Hamad Medical Corporation, said Qatar’s ageing population has meant there is a new emphasis on geriatric medicine, with elder care specialists now working as part of multi-disciplinary teams operating across HMC’s network of facilities.

“The geriatrics team works across a number of HMC facilities and services. Members of our team work in emergency, inpatient, and outpatient facilities and we collaborate with colleagues across the system. As an example, we work closely with our orthopedic colleagues to address the unique care needs of elderly patients who are recovering from fractures,” said Dr Abdulla.

“Rumailah Hospital also offers a wide range of geriatric services through its specialized long-term care units and its specialized falls prevention, memory, general geriatrics, and rapid assessment clinics,” he added.

Dr Abdulla said that while geriatric care is provided across HMC, services are integrated and there is a focus on treating the ‘whole patient’, taking every aspect of their life into account, including culture and tradition. He says many geriatric patients are managing multiple medical problems, including a decline in memory, injuries from falls, and weight loss.

“With approximately 12 percent of the country’s population projected to be over the age of 65 by 2022, demand for specialized geriatric services is expected to continue rising. Individuals over the age of 65 have a range of unique healthcare requirements that include a number of conditions typical of, if not specific to, aging, such as dementia, depression, delirium, incontinence, vertigo, falls, bone fractures, and nutritional deficiencies.

Elderly patients may also require longer hospital stays,” said Dr Abdulla.

Dr Abdulla said that HMC is continuing to add new and innovative services to address the specific care needs of elderly patients and he notes that current services, such as a memory clinic and a falls prevention clinic, have filled an important void. He says geriatric physicians also work across the system within multi-disciplinary teams, providing physical and occupational therapy, speech therapy, nutrition and dietetics services, and other allied healthcare services.

“We recently introduced a new service at Al Wakrah Hospital that provides specialised counselling for our elderly patients via a weekly outpatient clinic. This service is also provided at Enaya Specialized Care Centers 1 and 2 in Hamad Bin

Khalifa Medical City and involves providing patients with a comprehensive health assessment as well as any necessary follow-up care, including rehabilitation,” said Dr Abdulla.

While most patients with complex medical conditions are admitted to Rumailah Hospital’s long-term care wards, adult ventilator units, or the Enaya Specialized Care Centers, he notes that a large number of elderly patients are cared for by HMC’s Home Health Care Service.

“Our top priority in the management of elderly patients is placing the patient and his or her family at the center of the decision-making process. We are committed to providing patients with the information they need to be active participants in their care and treatment plans. All efforts are made to ensure the patient maintains maximum independence and can remain in the community,” added Dr Abdulla.

The groundbreaking Enaya Specialized Care Centers were opened in 2015 as part of HMC’s efforts to meet the needs of the country’s growing elderly population. The facilities are connected via a link bridge, allowing for efficiencies in staffing, equipment, and space. They provide around-the-clock monitoring and treatment for patients with complex care needs.

ويتم تقديم خدمات رعاية كبار السن في مؤسسة حمد الطبية من خلال عدة مستشفيات منها مستشفى حمد العام، حيث يتواجد في قسم الطوارئ أطباء من قسم كبار السن وطب الشيخوخة للتعامل مع حالات كبار السن، إلى جانب أطباء الشيخوخة للتعامل مع الحالات الحادة وفريق آخر للتعامل مع حالات الكسور بالتعاون مع أطباء قسم العظام.

وفي مستشفى الرميلة تتوفر خدمات كبار السن في قسم العيادة الخارجية من خلال العيادات الاستشارية التخصصية التي يتواجد فيها استشاريون على درجة عالية من الخبرة الطبية في هذا المجال مثل عيادة عدم الاتزان والسقوط وعيادة الذاكرة وعيادة الشيخوخة العامة وعيادة العلاج السريع لتقييم الحالات بصورة عاجلة، إلى جانب توفير خدمات تخصصية لكبار السن ممن يحتاجون الى الاقامة المطولة بالمستشفى مثل حالات التنفس الصناعي وحالات الغيبوبة وحالات التنفس عن طريق فتحة في القصبة الهوائية مع وجود فريق طبي متكامل ومتخصص للعناية بهذه الحالات وتلبية احتياجاتها الصحية والاجتماعية.

وأوضح الاطباء بمؤسسة حمد الطبية أن كبار السن (الفئة العمرية التي تبلغ 65 سنة فما فوق) يتعرضون خلال هذه المرحلة لما يعرف بمتلازمات الشيخوخة وهي عبارة عن مجموعة من الأعراض أو الحالات المرضية كثيرة الحدوث عند المسنين ولذلك يمتاز المتخصصون في رعاية هذه الفئة بمعرفة كيفية تشخيصها وعلاجها بطريقة سليمة وباستخدام فريق صحي متكامل ومختص في العيادات الخارجية أو المستشفيات أو في منازل المرضى كبار السن.

وقال الدكتور أمير إبراهيم عبدالله استشاري بقسم كبار السن وطب الشيخوخة بمؤسسة حمد الطبية، إن هناك الكثير من متلازمات الشيخوخة مثل عدم الاتزان التي تعرض كبار السن لحوادث السقوط أثناء المشي ما يسبب لهم كسورا عظمية وكذلك الوهن أو ما يعرف بمتلازمة الضعف والسلس البولي وضعف الذاكرة والهذيان الى جانب ألزهايمر ونقص التغذية.

وأوضح أن فئة كبار السن تتطلب البقاء في المستشفى مدة أطول للعلاج مقارنة بالفئات العمرية الأخرى كنتيجة للتعامل مع المضاعفات الناشئة عن الاصابة مثلا بكسر عظمي أو مرض آخر قد يستغرق علاجه مدة أقل لمريض شاب.

واشار الى أن مستشفى الوكرة أطلق مؤخرا خدمة الاستشارات التخصصية للمسنين من خلال عيادة أسبوعية في قسم العيادات الخارجية، بينما تم في مدينة حمد بن خليفة الطبية توفير خدمات كبار السن من خلال مركز عناية 1 وعناية 2 حيث يتم إجراء تقييم صحي شامل للمسنين وإعادة تأهيلهم من خلال فريق طبي متكامل يضم استشاريين في طب المسنين وأخصائيي العلاج الطبيعي والوظائفي وأمراض التخاطب والبلع والصيادلة السريريين والتغذية، إلى جانب كادر تمريضي متميز.

وتقوم مؤسسة حمد الطبية بنقل الحالات المرضية من كبار السن (التي يمكنها مغادرة المستشفى بعد إتمام العلاج ولكن يتعذر نقلها إلى المنزل) الى مركز (عناية) في مدينة حمد بن خليفة الطبية والذي تتواجد فيه فرق علاجية متكاملة تابعة لإدارة كبار السن والرعاية المطولة الى جانب عدد كبير من الغرف المجهزة بأحدث المعدات والمستلزمات التي تتطلبها كل حالة مرضية حيث يتولى مدير الحالة تحديد الخطة العلاجية بالتنسيق والمتابعة مع الفريق العلاجي وتشمل خطة العلاج علاجا طبيا وتأهيليا وعلاجا طبيعيا وتأهيلا ذهنيا للمريض.
