4 health reasons that prevent diabetics from fasting

Doha: Qatar Diabetes Association, a member of Qatar Foundation for Education, Science and Community Development, held three educational sessions on diabetes and fasting, from a series of diabetes tent sessions it organizes weekly throughout the holy month of Ramadan.

The session, which was held via visual communication technology entitled “Fasting and Diabetes”, during which Dr. Mahmoud Zari, senior consultant and head of the Department of Endocrinology and Diabetes at Hamad Medical Cooperation, addressed the definition of diabetes and groups that should not fast, as Islam is a religion of ease, highlighting the importance of the person’s knowledge of diabetes by knowing the symptoms of low and high blood sugar,

That a person should break fasting if he feels these symptoms, such as trembling, accelerated heartbeat, or symptoms of high blood sugar such as extreme thirst and frequent going to the bathroom.

He also noted the importance of repeating the measurement or examination of sugar at least 4 times, first before iftar between 4:00 and 5:00 p.m. where low sugar can occur, secondly measuring sugar two or three hours after iftar, third before suhoor and fourth at 10 a.m. to make sure that the drug consumed at suhoor keeps sugar at the required rate.

Nada al-Khatib, nutritionist and health educator at the Qatar Diabetes Association, addressed the eating habits of people diagnosed with diabetes during Ramadan. There are changes in the quality, quantity and time of meals during the month of fasting, and advice was provided: to reduce the amount of starches consumed, avoid starting your iftar with juices and water, reduce the intake of dates so that you can eat from one to three dates according to the reading of your blood sugar, increase the intake of salad with a bowl of soup.

She also stressed the importance of distributing meals so that the intake of starches is not concentrated in one meal, taking care to repeat glucose meters checks during the day and after iftar to make sure that the treatment is suitable with fasting and with eating habits that have become different during Ramadan and even around bedtime,

She stressed the care to have the suhoor meal while delaying it as much as possible, and to eat an appropriate amount of starches that should be discussed with a dietitian. Be careful to drink water to prevent dehydration, and the need to distribute water drinking from iftar to suhoor and not focus only on the pre-dawn period.

In the second tent session in the second week of Ramadan entitled “Pregnancy and Diabetes”, Dr. Mohammed Bashir, Senior Consultant endocrinologist and diabetes and head of the Diabetes Clinic at the Center for Women’s Health and Research, spoke about the types of diabetes for pregnant women, gestational diabetes, which occurs during pregnancy, and pre-pregnancy diabetes, both type 1 and 2, which are originally diagnosed with pregnant women.

He explained the danger of fasting to the pregnant woman diagnosed with diabetes, because it is dangerous for the mother and fetus for fear of running out of glucose from the body of the pregnant woman, after which the body begins to break down fat and protein to be used as an alternative to glucose, and here ketone acids arise, and if they are at higher rates than the average rate, they will become very harmful to the fetus.

Dr. Bashir also advised diabetic pregnant women to plan at least three months before pregnancy, as their Glycemic index must be under 7% so that there are no fetal abnormalities, and 5 mg of Folic acid pills should be taken three months before pregnancy for the health of the fetus’s spinal cord later.

At the third session in the third week of Ramadan, entitled “Primary Health Care Clinics and Services”, Dr. Khaled al-Karbi, a family medicine specialist at the Primary Health Care Corporation, addressed the services provided by the Foundation in detail.

الدوحة: عقدت الجمعية القطرية للسكري، عضو مؤسسة قطر للتربية والعلوم وتنمية المجتمع، ثلاث جلسات تثقيفية حول السكري والصيام، من سلسلة جلسات خيمة السكري التي تنظمها أسبوعيًا طوال شهر رمضان المبارك.

جاءت الجلسة، التي عُقدت عبر تقنية الاتصال المرئي بعنوان «الصيام و السكري»، تناول خلالها الدكتور محمود زرعي استشاري أول ورئيس قسم الغدد الصمّاء والسكري بمؤسسة حمد السكري، التعريف بمرض السكري والفئات التي لا يجب أن تصوم، حيث إن الدين يُسر، مُسلطًا الضوء على أهمية إلمام المُشخّص بالسكري بمعرفة أعراض انخفاض وارتفاع نسبة السكر في الدم، حيث يجب عليه كسر الصيام في حالة الشعور بهذه الأعراض مثل الارتجاف أو تسارع ضربات القلب أو الشعور بأعراض الارتفاع مثل العطش الشديد وتكرار الذهاب للحمّام.

كما نوه بأهمية تكرار قياس أو فحص السكر على الأقل 4 مرات، أولًا قبل الإفطار ما بين الساعة الرابعة والخامسة عصرًا حيث يمكن أن يحدث انخفاض السكر، ثانيًا قياس السكر بعد الإفطار بساعتين أو ثلاثة، ثالثًا قبل السحور ورابعًا عند العاشرة صباحًا للتأكد من أن الدواء المُتناول عند السحور يُبقي نسبة السكر في المُعدل المطلوب.

وتناولت السيدة نداء الخطيب، أخصائية التغذية والمُثقفة الصحية بالجمعية القطرية للسكري العادات الغذائية للمُشخّصين بالسكري خلال رمضان. وأن هناك تغيّرات في نوعية الوجبات وكميتها ووقتها خلال شهر الصيام، وقدّمت نصائح منها: الحرص على تقليل كمية النشويات المُتناولة، تفادي العصائر و كسر الصيام بالماء، تقليل تناول التمر بحيث يمكن تناول من حبة إلى ثلاث حبات تمر وفقًا لقراءة نسبة السكر، الإكثار من السلطة مع تناول صحن الشُرْبة.

كما أكدت على أهمية توزيع الوجبات بحيث لا يتركز تناول النشويات في وجبة واحدة، مع الحرص على تكرار فحص نسبة السكر خلال اليوم وبعد الإفطار للتأكد من أن العلاج مُناسب مع الصيام ومع العادات الغذائية التي أصبحت مُختلفة خلال رمضان وحتى أوقات النوم، وشدّدت على الحرص على تناول وجبة السحور مع تأخيرها على قدر المُستطاع، وتناول قدر مُناسب من النشويات والتي يجب مُناقشتها مع أخصائية التغذية.. كما يجب الحرص على شرب الماء منعًا للتعرّض للجفاف، وضرورة توزيع شرب الماء من الإفطار وحتى السحور وليس التركيز على فترة ما قبل الإمساك فقط.

وفي جلسة الخيمة الثانية في الأسبوع الثاني من رمضان وبعنوان «الحمل و السكري» تحدث الدكتور محمد بشير، استشاري أول الغدد الصمّاء والسكري ورئيس عيادة السكري بمركز صحة المرأة والأبحاث، عن نوعي السكري بالنسبة للحامل، سكري الحمل، والذي يطرأ أثناء الحمل، وسكري ما قبل الحمل سواء النوع الأول أو الثاني والذي تكون أصلًا الحامل مُشخّصة به.

وأوضح خطورة الصيام على الحامل المُشخّصة بالسكري، وذلك لخطورته على الأم والجنين خوفًا من نفاد الجلوكوز من جسم الحامل والذي يبدأ بعده الجسم في تكسير الدهون والبروتين ليستخدمها كبديل للجلوكوز، وهنا تنشأ أحماض الكيتون ، وإذا كانت بمعدلات أكبر من المُعدّل فستصبح ضارة جدًا بالجنين.

كما نصح الدكتور بشير، الحوامل المُشخّصات بالسكري بالتخطيط على الأقل ثلاثة أشهر قبل الحمل، حيث يجب أن يكون مخزون السكر لديها تحت 7% حتى لا تحدث تشوهات للجنين، كما يجب أخذ 5 ملغ من حبوب فولك أسيد قبل الحمل بثلاثة أشهر وذلك لصحة النخاع الشوكي للجنين لاحقًا. أما في الجلسة الثالثة في الأسبوع الثالث من رمضان، وتحت عنوان (الرعاية الصحّية الأولية العيادات والخدمات)، تناول فيها الدكتور خالد الكربي، اختصاصي طب الأسرة بمؤسسة الرعاية الصحية الأولية، الخدمات التي تقدّمها المُؤسسة بالتفصيل.
