Doha: Qatar Rehabilitation Institute (QRI) has launched the first rehabilitation helpline “Tawasol” designed to make it easier for discharged patients to access follow up rehabilitation care and services.
Covering pediatric, adult, and older adult rehabilitation services, the helpline is designed to simplify the process for patients to continue with their required rehabilitation care to support their recovery after leaving hospital.
Dr. Hanadi Al Hamad, Medical Director of QRI and National Lead for Healthy Ageing, explains the importance of this helpline in promoting effective continuity of care: “Every day our clinicians see patients who have experienced a physical trauma following an accident, or neurological damage following a stroke or major surgery, or they have a debilitating chronic disease that impacts their ability to function independently in the community.”
“Ongoing rehabilitation therapy is an important component of the patients recovery journey, not only during their hospital stay but also after they have been discharged from hospital. While we provide rehabilitative care to a broad spectrum of patients, we also have a significant number of patients who struggle to find their way within the healthcare system. For instance, some of our older patients experience more challenges in accessing the right follow up care after they have been discharged. Our helpline will help address these challenges not only will it help patients get follow up appointments for rehabilitative care but it will also enable patients aged 60 and above to get an appointment with the new Geriatric Wellness Clinic,” added Dr. Al Hamad.
Mashael Jassim, Assistant Executive Director of Nursing, has led the efforts to establish the dedicated helpline to facilitate easier access and improve continuity of rehabilitation care and services: “Our staff found that some patients experienced difficulties in continuing their rehabilitation care as outpatient clients after they were discharged. In some cases patients did not understand what was needed to initiate necessary after care and the language barrier made it more difficult for them to seek help through the regular channels.”
“Occasionally the unavailability of treatment slots resulted in delays that could potentially have serious consequences on the patients recovery. Continuity of care and care coordination are important concepts that make significant contributions to how people experience health and related care,” Ms. Jassim added.
Dr. Fatma Al Kuwari, Assistant Chairperson of the Physical Medicine and Rehabilitation Department is a key sponsor of this program, stated that: “The risk of delayed or discontinued rehabilitative care is that this may lead to relapse in the patients condition and possibly readmission to inpatient care. This is neither desirable or efficient for the patient and their family, nor for our facility so by enabling the right delivery of follow up care we can ensure better patient centered care and operational performance.”

الدوحة: أطلق مركز قطر لإعادة التأهيل التابع لمؤسسة حمد الطبية أول خط مساعدة مخصص للخدمات التأهيلية /تواصل/، بهدف تسهيل حصول المرضى على الرعاية المستمرة وخدمات إعادة التأهيل.
ويقدم خط المساعدة الخدمات التأهيلية لكل من الأطفال والبالغين وكبار السن، حيث صمم للتسهيل على المرضى لمواصلة تلقيهم للرعاية التأهيلية المطلوبة لمساعدتهم على التعافي بعد مغادرة المستشفى.
وقالت الدكتورة هنادي الحمد المدير الطبي لمركز قطر لإعادة التأهيل وقائد برنامج الشيخوخة الصحية في الاستراتيجية الوطنية للصحة، إن خط المساعدة تكمن أهميته في تعزيز الاستمرارية الفعالة للرعاية الصحية حيث يتابع الفريق الطبي يوميا عددا من المرضى الذين تعرضوا لإصابات جسدية بعد إصابتهم بحوادث، أو تلف عصبي بعد الإصابة بسكتة دماغية أو عملية جراحية كبيرة، أو يعانون من مرض مزمن ومنهك يؤثر على قدرتهم في القيام بدورهم بشكل مستقل في المجتمع.
وأشارت إلى أن العلاج التأهيلي المستمر يعد عنصرا مهما خلال رحلة تعافي المريض أثناء إقامته وبعد خروجه أيضا من المستشفى، ففي حين يتم تقديم الرعاية التأهيلية لمجموعة واسعة من المرضى، يوجد أيضا عدد من المرضى الذين يحتاجون لمزيد من التسهيلات داخل نظام الرعاية الصحية مثل المرضى من كبار السن الذين يحتاجون الرعاية الصحيحة بشكل مستمر بعد خروجهم من المستشفى، حيث يقوم خط المساعدة بمساعدتهم على تخطي أي صعوبات، كما يمكن المرضى الذين تبلغ أعمارهم 60 عاما وما فوق من الحصول على موعد مع عيادة المعافاة لكبار السن الجديدة بجانب حصولهم على مواعيد المتابعة المتعلقة بالرعاية التأهيلية.
من جهتها قالت السيدة مشاعل جاسم مساعد المدير التنفيذي للتمريض، التي تولت قيادة الجهود المبذولة لإنشاء خط المساعدة المخصص لتسهيل حصول المرضى على الرعاية وتحسين استمرارية خدمات إعادة التأهيل، إن بعض المرضى يواجهون صعوبات في مواصلة رعايتهم التأهيلية كمرضى خارجيين بعد خروجهم من المستشفى حيث أنه في بعض الحالات قد لا يعرف المرضى ما هو مطلوب لبدء الرعاية الضرورية بعد العلاج ، كما أن حاجز اللغة قد يصعب على المرضى طلب المساعدة من خلال الطرق العادية.
ولفتت إلى أنه في بعض الأحيان قد يؤدي تأخر العلاج إلى التأثير على تعافي المريض، كما أن استمرارية الرعاية وتنسيقها يعدان من المفاهيم المهمة التي تساهم بشكل كبير في تحسين تجربة المرضى فيما يتعلق بالصحة والرعاية.
وبدورها قالت الدكتورة فاطمة الكواري مساعد رئيس قسم الطب الفيزيائي والتأهيل، إن تأخر حصول المريض على الرعاية التأهيلية أو توقيفها قد يؤدي إلى حدوث انتكاس في حالة المريض وربما إعادة إدخاله إلى المستشفى كمريض داخلي، وهو أمر غير مرغوب فيه من قبل المريض وعائلته وغير فعال للمرفق الصحي لذلك يمكن ضمان تحقيق رعاية أفضل وأداء تشغيلي أعلى يركز على المريض من خلال تقديم خدمة متابعة الرعاية.
