5 ways to issue a student exemption certificate and remote work

Doha: Yousef Al-Maslamani – Medical Director of Hamad General Hospital at HMC – revealed an update of the lists of patients registered with it who suffer from chronic diseases that require the issuance of medical certificates stating that they may face complications when contracting the Coronavirus (Covid-19) to work remotely in light of the new medical data on the (Covid-19) virus.

In this context, Dr. Al-Maslamani confirmed, in exclusive statements to Al-Sharq, that lists of chronic disease patients have been reviewed according to specialized committees from the Ministry of Public Health, Hamad Medical Corporation, and Primary Health Care Corporation, where the latest medical information about the (Covid-19) virus indicates that there is no difference. Its effect on patients with some chronic diseases compared to other people.

Dr. Al-Maslamani added, “The data of patients who meet the criteria for issuing a chronic disease certificate have been placed in the electronic system of HMC and PHCC during the past week, so that the certificate can be issued upon request in an easy and quick manner, and these certificates are also officially approved by all parties and contain the barcode that shows the name of the patient to whom the certificate was issued and his ID number.

Dr. Al-Maslamani indicated that a certificate proving that you have a chronic disease can be obtained through “My Health” portal. Online certificates are available through this portal ready-made certificates for chronic disease patients who meet the new standards and who are receiving treatment in Hamad Medical Corporation facilities or Primary Health Care Corporation facilities. The Certification can also be obtained through an online certificate request form. Patients with chronic diseases who hold a health card and are receiving treatment at HMC facilities or PHCC facilities can apply for a certificate using the online certificate application form, and the certificate is prepared and sent to the patient applying by e-mail within 7 days from the date of submitting the application, as well patients can complete the application form available on the Primary Health Care Corporation website for all registered patients who hold a valid health card.

The Ministry of Public Health website explained the mechanism for obtaining the certificate through the electronic portal “MyHealth”, by uploading the certificate via the link:

As available on “MyHealth” electronic portal, certificates are ready for chronic disease patients who are receiving treatment in HMC facilities or PHCC facilities. Chronic diseases patients who have not registered in the online portal must initiate registration at the portal immediately enables them to obtain the required medical exemption certificate, and the registration and activation process for this service takes about 24 hours. Patients with chronic diseases who hold a health card and are receiving treatment at HMC facilities or PHCC facilities can request to obtain a certificate using an application form. This is available for issuance of the certificate through the Primary Health Care Center. Patients with chronic diseases who have a Qatari health card but have never received treatment in facilities of HMC or PHCC facilities (i.e., chronic disease cases are not documented in their medical records in these facilities) Obtaining an appointment at the Primary Health Care Center to register their chronic disease cases. Patients can book an appointment and submit a request at the health center to obtain This certificate, but in the case of a private or semi-private health center, patients with chronic diseases who do not hold a Qatari health card must go to one of the private health centers in the State of Qatar and ask it to provide them with medical exemption certificates.

وفي هذا السياق أكد الدكتور المسلماني في تصريحات خاصة لـ”الشرق” أنه تم مراجعة قوائم مرضى الأمراض المزمنة وفق لجان متخصصة من وزارة الصحة العامة، ومؤسسة حمد الطبية، ومؤسسة الرعاية الصحية الاولية حيث تفيد المعلومات الطبية الأحدث حول فيروس (كوفيد-19) بعدم اختلاف تأثيره على مرضى بعض الأمراض المزمنة مقارنة بغيرهم من الأشخاص.

وأضاف الدكتور المسلماني قائلاً” لقد تم وضع بيانات المرضى الذين تنطبق عليهم معايير إصدار شهادة المرض المزمن في النظام الإلكتروني لمؤسسة حمد الطبية ومؤسسة الرعاية الصحية الأولية خلال الأسبوع الماضي بحيث يمكن إصدار الشهادة عند الطلب بصورة ميسرة وفي أسرع وقت، كما أن هذه الشهادات معتمدة رسمياً لدى كافة الجهات وتحتوي على الباركود الذي يوضح اسم المريض الذي صدرت له الشهادة ورقم بطاقته الشخصية”.

وأشار الدكتور المسلماني إلى أنه يمكن الحصول على شهادة تثبت الإصابة بحالة مرضية مزمنة عن طريق بوابة صحتي ” My Health” الإلكترونية حيث تتوفر من خلال هذه البوابة شهادات جاهزة لأصحاب الأمراض المزمنة الذين تنطبق عليهم المعايير الجديدة والذين يتلقون العلاج في مرافق مؤسسة حمد الطبية أو مرافق مؤسسة الرعاية الصحية الأولية، كما يمكن الحصول على الشهادة عن طريق استمارة طلب شهادة عبر شبكة الإنترنت حيث يمكن لمرضى الأمراض المزمنة الذين يحملون بطاقة صحية ويتلقون العلاج في مرافق مؤسسة حمد الطبية أو مرافق مؤسسة الرعاية الصحية الأولية طلب الحصول على شهادة باستخدام استمارة طلب شهادة عبر شبكة الإنترنت، ويتمّ إعداد الشهادة وإرسالها إلى المريض المتقدّم بالطلب عن طريق البريد الإلكتروني خلال 7 أيام من تاريخ تقديم الطلب، كما يمكن للمرضى إكمال نموذج الطلب المتاح على الصفحة الإلكترونية الخاصة بمؤسسة الرعاية الصحية الأولية لجميع المرضى المسجلين ممن يحملون بطاقة صحية سارية المفعول.

حيث تتوفر في بوابة ” صحتي” ” MyHealth” الإلكترونية شهادات جاهزة لأصحاب الأمراض المزمنة الذين يتلقون العلاج في مرافق مؤسسة حمد الطبية أو مرافق مؤسسة الرعاية الصحية الأولية، ويتعين على المرضى الذين يعانون من الأمراض المزمنة الذين لم يقوموا بالتسجيل في البوابة الإلكترونية المبادرة إلى التسجيل في البوابة على الفور ليتمكنوا من الحصول على شهادة الإعفاء الطبي المطلوبة، وتستغرق عملية التسجيل والتفعيل لهذه الخدمة حوالي 24 ساعة، يمكن لمرضى الأمراض المزمنة الذين يحملون بطاقة صحية ويتلقون العلاج في مرافق مؤسسة حمد الطبية أو مرافق مؤسسة الرعاية الصحية الأولية طلب الحصول على شهادة باستخدام استمارة طلب شهادة يتمّ تنزيله عبر شبكة الإنترنت من خلال الرابط التالي:

يتمّ إعداد الشهادة وإرسالها إلى المريض المتقدّم بالطلب عن طريق البريد الإلكتروني خلال 7 أيام من تاريخ تقديم الطلب.، كما يتاح إصدار الشهادة عن طريق الموقع الإلكتروني لمؤسسة الرعاية الصحية الأولية يمكن للمرضى إكمال نموذج الطلب المتاح على الصفحة الإلكترونية الخاصة بمؤسسة الرعاية الصحية الأولية لجميع المرضى المسجلين ممن يمتلكون بطاقة صحية سارية المفعول.

هذا ويتاح إصدار الشهادة عن طريق مركز الرعاية الصحية الأولية، فيتعيّن على مرضى الأمراض المزمنة الحاصلين على بطاقة صحية قطرية ولكن لم يسبق لهم أن تلقوا العلاج في مرافق مؤسسة حمد الطبية أو مرافق مؤسسة الرعاية الصحية الأولية (أي أن الحالات المرضية المزمنة غير موثّقة في سجلاتهم الطبية في هذه المرافق) الحصول على موعد لدى مركز الرعاية الصحية الأولية لتسجيل الحالات المرضية المزمنة الخاصة بهم، يمكن للمرضى حجز موعد وتقديم طلب في المركز الصحي للحصول على هذه الشهادة، أما في حال المركز الصحي الخاص أو شبه الخاص يتعيّن على مرضى الأمراض المزمنة ولا يحملون بطاقة صحية قطرية التوجّه إلى أحد المراكز الصحية الخاصة في دولة قطر والطلب منه تزويدهم بشهادات إعفاء طبي.
