HMC, Sidra Medicine collaboration saves life of newborn with rare brain condition

A team of experts from Hamad Medical Corporation’s (HMC) Hamad General Hospital (HGH) and Neuroscience Institute and Sidra Medicine has saved the life of a newborn baby born with a rare brain condition.
Led by a neuro-interventional radiologist and his team from Hamad General Hospital, in collaboration with Sidra Medicine, the life-saving procedure was performed when the baby was less than two weeks old.
The Qatari baby boy suffered from Vein of Galen Malformation (VOGM), a rare condition that occurs before birth and results in abnormal connections between blood vessels within the brain and associated congenital heart defects. The condition, which occurs in one to two percent of vascular malformations of the brain, was detected during a routine scan by the team at Sidra Medicine late in the mother’s pregnancy.
To protect the baby and reduce the risk of complications during the final weeks of pregnancy, the baby was delivered three weeks early by caesarian section at Sidra Medicine. An extensive multidisciplinary team of neuro and interventional radiologists, cardiac interventionalists, neurosurgeons, and neonatologists was assembled to oversee the newborn’s care.
Dr. Ayman Zakaria, Senior Consultant, Interventional Neuroradiology at Hamad General Hospital, and the Neuroscience Institute, along with his team, led the procedure.
“Babies with Vein of Galen Malformation normally develop the condition during the first trimester of pregnancy. It results in abnormal arteries that connect directly into the precursor of the Vein of Galen instead of connecting with capillaries. This leads to extra pressure in the vein. It can bleed or dilate to prevent the drainage of the fluids inside the brain or cause a rush of blood towards the heart and lungs, causing extreme pressure on the right side of the heart, leading to congestive heart failure,” said Dr. Zakaria.
The first procedure, which took five hours, was performed by Dr. Zakaria and his team and supported by Dr. Walid Mubarak, a senior attending physician from Sidra Medicine’s Diagnostics Imaging Unit. The procedure was conducted at Sidra Medicine’s world-class intra-operative imaging suite (IMRIS); a hybrid operating theater featuring intraoperative imaging capabilities with the capacity to run complex interventions within the same setting.
The first procedure was a success and was followed by a second procedure a week later to further improve the baby’s heart circulation. The second procedure was also performed by Dr. Zakaria and his team from Hamad General Hospital, with the support of Dr. Ashley Robinson, Division Chief of Interventional Radiology at Sidra Medicine.
The baby is recovering well at Sidra Medicine’s Neonatal Intensive Care Unit and is showing marked improvement in his cardiac and vital signs. Another follow-up session will be performed by Dr. Zakaria when the baby is around two months old. This procedure will complete the closure of the remaining shunts and ensure normal physical development and brain function.

نجح فريق من جراحي مستشفى حمد العام، ومعهد العلوم العصبية، التابعين لمؤسسة حمد الطبية ومن سدرة للطب، في إجراء عملية بالغة الدقة لطفل حديث الولادة لم يكمل أسبوعين من عمره، يعاني من مرض دماغي نادر.
وقد أشرف على العملية التي تكللت بالنجاح، أخصائي في الطب العلاجي العصبي بالأشعة التداخلية في مستشفى حمد العام، إثر تشخيص حالة الطفل التي تبلغ نسبة حدوثها 1-2 % من حالات التشوه الخلقي الدماغي، أثناء الفحص الروتيني لوالدة الطفل في المراحل الأخيرة من الحمل من قبل فريق طبي في «سدرة للطب».
وكان الطفل يعاني من تشوه في وريد «جيلين»، وهو تشوه يحدث للجنين يؤدي إلى تداخل غير طبيعي بين الأوردة والشرايين الدموية داخل الدماغ، إضافة إلى تشوهات وراثية في القلب مرتبطة بهذه الحالة المرضية، وأجرى أطباء «سدرة للطب» عملية ولادة قيصرية لوالدته قبل موعد الولادة بثلاثة أسابيع لحمايته من مخاطر المضاعفات المرتبطة بحالته الصحية.
وقال الدكتور أيمن زكريا استشاري أول الطب العلاجي العصبي بالأشعة التداخلية في مستشفى حمد العام، ومعهد العلوم العصبية، الذي أشرف مع فريقه على العملية، إن علامات هذه الحالة النادرة المسماة «تشوه وريد جيلين» تظهر عادة في الشهور الثلاثة الأولى من الحمل حيث يؤدي هذا التشوه إلى ترابط الشرايين الدموية مباشرة مع وريد جيلين بدلاً من الترابط مع الشعيرات الدموية الدماغية، الأمر الذي ينجم عنه زيادة في الضغط في وريد جيلين ويمكن أن يتعرض المريض معه للنزيف الدموي الدماغي أو لتوسع كبير في الوريد يصعب معه تصريف السوائل من الدماغ.
كما يمكن أن يتسبب ذلك في تدفق مفرط في الدم باتجاه القلب والرئتين مما يضع كماً كبيراً من الضغط على الجانب الأيمن من القلب، ويتسبب فيما يعرف بالقصور القلبي الاحتقاني.
وقد قام بإجراء الجزء الأول من العملية العلاجية الذي استغرق خمس ساعات الدكتور زكريا يسانده فريق من مستشفى حمد العام، إضافة إلى الدكتور وليد مبارك الطبيب الاستشاري في وحدة التصوير الطبي الإشعاعي في «سدرة للطب»، حيث تم إجراء العملية في وحدة العلاج التداخلي الإشعاعي في «سدرة للطب» وهي عبارة عن غرفة عمليات هجينة متطورة مخصصة لعمليات العلاج التداخلي بالغة التعقيد ومجهزة بتقنيات تصوير إشعاعي مباشر وحي أثناء العمليات.
كما تم إجراء المرحلة الثانية من العملية التي تهدف إلى تحسين الدورة الدموية القلبية لدى الطفل المريض بعد أسبوع من المرحلة الأولى، حيث قام بهذه المرحلة الدكتور زكريا وفريقه، بمساندة من الدكتورة آشلي روبنسون رئيس قسم العلاج التداخلي الإشعاعي في «سدرة للطب».
ومن المقرر أن يتم إجراء المرحلة الثالثة من العملية العلاجية عند بلوغ الطفل شهره الثاني، لاستكمال إقفال التحويلات الوريدية المتبقية والتحقق من الحالة النمائية والوظيفية الطبيعية للدماغ.
من جانبه قال الدكتور أحمد عون رئيس معهد العلوم العصبية، إن الفريقين الطبيين من مؤسسة حمد الطبية و»سدرة للطب»، راضيان عما تم تحقيقه من نتائج علاجية ناجحة لهذه العملية المعقدة، خاصة أن معالجة هذه الحالة النادرة والمهددة للحياة كانت تنطوي على نسبة كبيرة من المخاطر، حيث يرجع الفضل في نجاح العملية إلى التعاون العلمي والطبي المتواصل بين الجانبين.
بدوره أفاد الدكتور عبدالله الأنصاري رئيس الإدارة الطبية بمؤسسة حمد الطبية، بأن مزودي الرعاية الصحية عازمون على مواصلة تقديم أفضل مستوى ممكن من الرعاية الصحية وفق أعلى المعايير الدولية للمرضى في دولة قطر، وذلك على الرغم من الظروف غير المسبوقة التي يمر بها العالم جراء جائحة كورونا «كوفيد – 19».
من جهته، نوه البروفيسور زياد حجازي الرئيس الطبي بالوكالة في «سدرة للطب» بتعاون المركز مع مستشفى حمد العام ومؤسسة حمد الطبية ومعهد العلوم العصبية، والذي ساهم في توفير مستوى راقٍ من الرعاية الصحية لأطفال دولة قطر، كما كان لمنهجية الفرق متعددة التخصصات الطبية المتبعة في الجهتين دور محوري في تقديم الرعاية الصحية التي يحتاجها هذا الطفل وإنقاذ حياته.
